IRS FORMS
The IRS recently issued the revised 2022 Form W-4P (Withholding Certificate for Periodic Pension or Annuity Payments), however, the IRS is postponed the requirement to begin using the forms until January 1, 2023.
MCFRS Retired ID Card
To obtain a Retired ID card please send and email that includes the following
* Rank at time of retirement
* Last 4 Digits of SSN
* Full Mailing Address
* Full Name
* A current picture with a smart phone (Similar to driver’s license) No pictures or
decoration in the background. Neutral color background. No hats or Glasses
